•    Application form (can be filled on the PC screen or to print out and to fill by hand, more detailed instructions for filling the application form. Must be signed by the applicant); 
•    Annex of the application form (can be filled on the PC screen or to print out and to fill by hand); or 
•    Valid travel document of the applicant;
•    Colour photo of minimum size of 35x45 mm;
•    Document/-s proving cause of extension of period of stay (for example, notice of a competent medical institution); 
•    Documents proving payment of state fees;
•    Identity document of the person signing the application form, if an alien has not signed the application form personally;
•    Identity document of the person submitting the application form, if an alien has not submitted the application form personally;
•    Medical expenses insurance policy valid in Estonia for a minimum limit of indemnity of 30 000 EUR (469 000 Estonian kroons) for the whole period of stay in Estonia.
