We emphasize that upon applying for passport, in addition to an application form, you need to submit other documents, too.

An application form must be filled in with block letters in Estonian language, and no corrections must be there.

An application form can be filled in on a computer. We recommend you to open the application forms with Adobe Acrobat Reader. If an application is submitted by post, then you have to print out and sign the application form, and a photograph must be glued to it.

You can fill in an application form with your own hand if you wish. You should use a black pen or ink pen which would provide a good preservation of data in the application form. You have to fill in all the data fields to be filled in by an applicant.


Signature
An applicant must fill in a signature field since he/she is 15 years old.
An applicant has to put his/her signature* in the cell set at the upper edge of an application form using a black pen or ink pen. A signature must be done with an even line. You have to check that the line of signature would not be lighter or broken in any places, and no unnecessary dots, little lines, corrections etc are acceptable.


Signature must not exceed the provided bounds! If a signature is put in an application form incorrectly, then it cannot be entered in the document.


*If an application for a child of 15-18 years old is submitted by his/her legal representative, then the child shall put his/her signature in the cell set at the upper edge of an application form, and the legal representative shall sign at the end of the application form to confirm the submitted data.


A child of 7-14 years old or adult with restricted active legal capacity can submit a signature specimen, but it is not compulsory. In case of a child under 7 years old or person unable to sign the signature field remains empty.


An applicant's personal data
You must submit your exact personal data. If an application is submitted for a child, then the child's personal data must be entered.
If a person's name contains foreign language characters, then the person's name shall be entered in a document according to the transcription rules of the International Civil Aviation Organization (ICAO) and, if possible, the original letters shall be retained. (Identity Documents Act, § 91).
Place of birth is marked within country (e.g. Estonia).


An applicant's contact data
Contact data whereto notices can be sent by post have to be marked in an application form. (Registered place of residence is not important.)


Issuance of document
Document can be received at a suitable Citizenship and Migration Service Office of Prefecture. Please mark the most suitable service office, and code and place of issuance, e.g. 743 Kuressaare.


An applicant's legal representative
This data is entered in case of submission of an application for document for a child under 18 years old or person with restricted active legal capacity is submitted by his/her legal representative. Please mark hereby whether an application is submitted by a parent or guardian.
