Questionnaires for applying for an ID-card for the Estonian citizens:
•    Application form to be filled out on a computer
•    Application to be filled out by hand
 
 
The questionnaire has to be filled out, above all, for applying for documents by post or by e-mail.  NB! In addition to application form, several other documents have to submitted, the list of documents can be checked here.  

An application form has to be filled out in block letters in the Estonian language and it must not contain any corrections.
 
It is possible to fill out the application form on a computer. For opening of the application forms, we suggest to use Adobe Acrobat Reader. If the application will be submitted by post, then it is needed to print a hard copy of an application, to sign it and to affix a photo to the application. In order to submit the application by e-mail, save the application form in the computer, the photo, specimen signature in *jpg format and other documents into the container and sign the application form electronically. The application from can be filled out by hand, if so wished. Black pen or ink pen has to be used in order to secure storing of the data of the hard copy. All compulsory data fields have to be filled in.

Signature
Specimen signature shall be submitted by an applicant of at least 15 years of age.
An applicant will write his or her signature* in the cell at the upper edge of an application form using a black pen or ink pen. A signature must be done with an evenly strong line. It is necessary to check that the line of signature would not be lighter or broken in any places, there were no unnecessary dots, dashes, corrections etc.  

Signature must not exceed the provided limits! If a signature is written in an application form inaccurately, it cannot be entered into the document.

*If an application for a child of 15-18 years is submitted by his or her legal representative (parent), the child will write his or her signature in the cell at the upper edge of an application form, and the legal representative will sign at the end of the application form to confirm the submitted data.

A child of 7-14 years old or adult with restricted active legal capacity can submit a signature specimen, but it is not compulsory. In case of a child under 7 years old or person unable to sign the signature field remains empty.

By applying for an ID-card, a marking is made in the box before the ID-card. If passport and ID-card are applied for together, then marking is made into both boxes. Then the reason for applying for document(s) is clarified by a marking (By applying for a relevant document for the first time, the reason for applying is marked as „primary document“).

Applicant’s personal data
One’s exact personal data should be written. If document is applied for a child, then a child’s data are entered. If a person’s name contains foreign letters, the person’s name shall be entered in a document according to the transcription rules of the International Civil Aviation Organization (ICAO) and, if possible, the original letters shall be retained (section 91 of the Identity Documents Act).
Place of birth is specified at a country level (for example, Estonia).

Applicant’s contact data
Into the application form one’s contact data from where notifications by post can be received (registered place of residence has no importance).

Applicant’s parents
Personal data of applicant’s parents shall be entered by applying for a primary document (e.g. if to a person to whom the document is applied for, has not been issued an identity document of the Estonian citizen by CMB or PBGB earlier).

Issuance of the document
•    The document can be received from the convenient Service Office of the Citizenship and Migration Bureau of the Prefecture. The place and code of most suitable Service Office will be marked at the application form: 743 Kuressaare
•    If it is wished to receive the document abroad, this will be issued in foreign representation of the Republic of Estonia. The most convenient foreign representation has to be chosen from the list „Estonian foreign representations issuing documents“ and to write down the place of issuance to the application form, for example: FOREIGN REPRESENTATION MOSCOW (code field will be left empty in the application form).
NB! Honorary Consuls do not issue the ID-cards!

Applicant’s legal representative
The field is entered in application form, is the application is submitted by the legal representative of a child of under 18 years of age or for a person of restricted active legal capacity. Thereat it is marked whether application is submitted by a parent or guardian.

NB! Data submitted in application form have to be confirmed by a signature at the end of the application form in the field intended for it.
